Those interested in participating in the Healthy Recreation Program are kindly asked to:

1. Register on the website and fill in the Application Form

3. Make the payment by bank transfer to:

4. Send the bank transfer confirmation slip to our office in Zakopane by e-mail or by post.

Registration of Participants
Participants will be registered only after having sent the completed application form together with a copy of the confirmation of the advance payment by e-mail or by post to the OsteoMed office in Zakopane. In the case of e-mail applications, please include the date of the bank transfer.
We only accept applications with effectuated payments.
Please specify the type of classes you wish to attend and the name of the participant in the transfer details.

Bills
The bills that we issue can only contain the details of the buyer (name and surname or company name and address), which figure on the payment confirmation, that we receive from you. We therefore ask you to complete the bank transfer slips very carefully.
We issue bills for the month in which the payment was received.
We send bills if we are asked to do so. If you wish to receive a bill, please let us know by phone, e-mail or by post.

Confirmation of Application
After receiving the application and the payment confirmation, we will send you an e-mail or letter with detailed organisational information. For classes with a limited number of participants, the order of applications is decisive.

Cancellation
The Physiotherapeutic Centre reimburses 90% of the amount paid to those, who make their cancellation in writing at least 10 days before the beginning of classes. In the case of later cancellations, we do not reimburse the payment, unless the place is filled by someone on the waiting list. 

Cancellation of classes  
The Centre has the right to cancel classes if there are not enough participants or for other significant reasons. In this case, we reimburse the total amount paid by the participant.
